Kidney Center of Lubbock (Lubbock, TX) is seeking an EXPERIENCED HOME PROGRAM STAFF NURSE.SCOPE OF JOB: The Home Program Staff Nurse is a qualified professi! onal nurse responsible for the training of ESRD patients and their partners to perform dialysis treatments in the home. FULL TIME POSITION, M-F, 32 hrs a week, 8 hr shifts. DUTIES
The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties, and additional responsibilities ma! y be delegated as required. 1. Practice at all times within li! mitation established by law, regulation, or usual practice within the state and under supervision of Licensed Practical Nurse (LPN), Registered Nurse (RN) or a Medical Doctor (MD) as dictated by state law. 2. Provide training and education to potential home dialysis patients in the clinics; trains dialysis patients to perform self-dialysis or home dialysis with little or no professional assistance, and trains other individuals to assist patients in performing self-dialysis or home dialysis.3. Consult with the supervisor on all aspects of patient care and execute the plan of nursing care for each patient as directed.4. Document all patient care provided and contribute to team patient care planning and follow-up to achieve goals.5.Conform to the established policies and procedure of the facility. 6. Participates in direct patient care and home dialysis training.7. Provides education to patient and partner on the setup and priming of the home hemodialysis system.8. Educates pa! tient and partner on cannulation of the access.9. Educates patient and partner on monitoring during home dialysis, safety checks, and prevention of potential complications during treatment.10. Educates patient and partner in following physician prescription for home dialysis treatment. 11. Educates patient and partner in the administration of any medications orderred during treatment.12. Educates patient and partner in discontinuation of dialysis, return of blood, breakdown, cleaning and maintenance of the system.13. After education of items 1 through 7, documents successful completion of training with return demonstration and completion of a Competency Skills Checklist.14. Completes annual education as dictated by Policy and Procedure15. Communicates with patient/partner and identifies problems, symptoms or patient complaints to the attending physician. 16. Completes initial, annual and follow up assessments in order to assure the completion of care plans as required 17. P! articipates in Interdisciplinary care plan meetings to assure completio! n of care plans as required 18. Completes monthly clinic visits19. Completes initial home visits and follow up visits as needed. 20. Other duties as assigned
QUALIFICATIONS
Graduate of an accredited school of nursing and currently licensed as a Registered Nurse Training and/or experience (one year preferred) in an outpatient hemodialysis settingSuccessful completion of the Color Blind Test per CMS Conditions of Coverage to perform required water system testingCPR Certification The statements below describe the general nature and level of work being performed in this role.
