The Day Hab Director supervises Community Facilitators in assigned day habilitation ensuring that all aspects of individual services are provided in compliance with consumer PDP’s, applicable laws,
regulations, policies, and procedures.
Essential Duties and Responsibilities of the position include but are not limited to:
Monitor and review all data for service objectives, medication, and behavior intervention to insure completion and accuracy.
Facilitates inclusion in community activities, use of natural supports, social interaction, and participation in leisure activities and development of socially valued behaviors.
Assesses and makes suggestions regarding Team Member and individual interactions.
Insures financial re! sponsibility regarding all controllable expenses from the Day ! Habilitation including, but not limited to, payroll, office supplies, and groceries.
Performs all tasks of the Community Facilitator including, but not limited to, reinforcement of counseling and therapy activities, direct personal assistance with daily living activities of individuals (grooming, eating, and personal hygiene) and, provides assistance with medications and performs tasks delegated by a RN.
Supervises Community facilitators and all other Team Members scheduled to work at the Day Habilitation.
