Summary:
The Social Worker serves
as a member of the interdisciplinary team for the provision of psychosocial assessment, planning and care in order to maximize the comfort and health of patients and families consistent with the Kindred at Home policies and procedures.
The Social Worker is r
esponsible for identifying patient/family psychosocial needs and for providing supportive care and appropriate services in accord! ance with the attending physician’s orders and the interdisc! iplinary team plan of care. In addition,
The Social Worker completes initial and ongoing psychosocial assessments, direct casework and counseling services of patient/family/caregiver accurately, timely, and in a caring fashion.
The Social Worker iInitiates communication with other team members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
The Social Worker works collaboratively as a member of the interdisciplinary team
The Social Worker assists patient and family/caregivers in meeting their needs to maintain independence at home, find appropriate alternate living or caregiver arrangements, and/or provide additional support to patient and family/caregiver during changes in levels of care.
The Social Worker obtains data on physical, psychosocial, social, spiritual, cultural and financial factors that may influence patient/family health status and incorporate that data into the ! plan of care.
The Social Worker communicates updates, changes and information to family at least weekly.
The Social Worker provides support to primary caregivers, volunteers and employed caregivers to assist in their ability to provide care to the patient as indicated.
The Social Worker shares in providing 24-hour, seven-day-a-week coverage to patients/families when necessary.
The Social Worker maintains current licensure certification and continuing education requirements for position, as applicable.
Qualifications
Education:
Graduate of an accredited Bachelor’s Degree in Social Work program.
Masters Degree in Social Work preferred.
Licenses/Certification:
A Valid Social Worker license in the state employed is required as is current CPR certification and a current driver’s license.
Reliable transportation and current auto insurance is required.
Experience:!
A m
inimum of 2 years experience as a professional so! cial worker and preferably one year in a home health setting.
Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
Demonstrated ability to work independently and cooperatively as an effective team member.
Ability to communicate effectively with patients and their family members, and at all levels of the organization.
Ability to maintain confidentiality.
