Duties:
The LVN is technically proficient and self-directed in initiating, performing, and completing practical nursing care assignments to acutely and chronically ill medical patients whose nursing car! e needs are managed in a clinic setting. The LVN is a member of the outpatient nursing team and is responsible for observing, recording and reporting changes in the patient's condition as well as assisting in the development and revision of the patient-driven holistic care plan for life. Demonstrates active collaboration and communication with Teamlet(Patient, Primary Care Provider, and RN Care Manger) and the larger team (family/caregiver, internal and community based services and discplines involved in providing care to the patient). The LVN is a member of the larger team.

Qualifications & Evaluations

Graduate of a school of practical or vocational nursing approved by the appropriate State agency, and/or accredited by The National League for Nursing Accrediting Commission (NLNAC) at the time the proram was completed by the applicant OR has health care education in the military service which is accepted by the licensing body in the jurisdiction in which licensed and is qualifying for full LPN/LVN licensure OR has a license granted by a jurisdiction that does not require graduation from an approved school
possess a current, full, active and unrestricted licensure as a lic! ensed practical or vocational nurse in a State, Territory, or Commonwea! lth (i.e.., Puerto Rico) of the U.S. or in the District of Columbia
proficient in spoken and written English (LVN/LPNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)).
